County Nutrition Action Plan/Partnership (CNAP)
The goal of our County Nutrition Action Plan/Partnership (CNAP) is to facilitate coordination, cooperation, and collaboration on healthy community and policy, systems, and environmental change efforts in Santa Cruz county. Our CNAP groups meet on a regular basis (quarterly) and engage a variety of members to identify and address shared priorities to help maximize resources and outcomes benefiting low income community members. 

The goal of the Santa Cruz County CNAP is to improve the nutritional well-being of Santa Cruz County residents eligible for federal nutrition assistance programs by increasing fruit and vegetable consumption and physical activity. The target objectives are:

  • Maintain collaboration and communication among agencies dedicated to improving nutrition, food security, and physical activity levels among low-income residents.
  • Increase awareness of current nutrition education and physical activity programs aimed at achieving and maintaining a healthy lifestyle.
  • Provide nutrition and physical activity education opportunities and resources to target populations.
  • Maximize availability and utilization of all food and nutrition programs including but not limited to CalFresh, WIC, food banks, pantries and school meal programs.

The CNAP meets quarterly. Partners include Second Harvest Food Bank, WIC, Santa Cruz City School District, United Way, Parajo Valley Health Trust, Salud Para Le Gente, Santa Cruz County Human Services Department, family resource centers, Catholic Charities, Pajaro Valley School District and Head Start.
